Match Registration Form for 2011-12 Season

Match Registration Form

This Match Registration Form MUST be competed after every game and emailed to Trevor Ruthers, League Registration Secretary within 4 days of the date played.

Failure to do so will incur a fine of £5 and/or the club being dealt with as the Management Committee decide.

Re-arranged Fixtures Form

Re-arranged Fixtures Form

This form should be used for all re-arrangements of fixtures, including those that are brought forward.

BOTH clubs must complete the form and send or email to Mike Cook, Fixture Secretary AND Jim Spiers, Referees' Secretary.

The Youth Registration Transfer

Transfer Form

When a player moves from one club to another the Youth Registration Transfer Form must be printed out and completed as necessary. The player and both clubs have to sign and date the document and then send to Trevor Ruthers, League Registration Secretary, 123 Cainscross Road, Stroud, GL5 4HN. A £1 fee is charged for each transfer.
